Please bring a photo ID, your insurance card, a list of current medications, and any current glasses or contact lenses. If this is your first appointment with us please complete the patient health history form linked below and bring it with you to your appointment.

You should expect your eye exam appointment to last about 30-60 minutes, depending on the complexity.
